You have approval. Now you need delivery.

Let’s see whether we can provide the support you have been approved for.

Start with three practical things: the service, your suburb or postcode and the timing that works for you. That gives us enough to start a real conversation and work out the next practical step with you.

No long application. You do not need to have every detail in front of you. Start with what you know and we can talk through the rest.

The first check

Three things tell us whether there is a realistic next step.

Approval matters, but it is only one part of starting care. The service still needs to match the place, timing and people available to deliver it well.

1

What support?

Tell us the service or services you have approval for. You do not need to translate your paperwork into perfect terminology — plain language is fine.

2

Where?

Your suburb or postcode helps us understand where you are and whether our team is currently working in your area.

3

When?

Preferred days, times and how soon you would like support to start help us test the request against real roster capacity rather than a vague promise.

If the three line up: we can move into a service-planning conversation and work through the details together. If something does not line up: we will explain that clearly and talk about the most useful next step.
What happens next

A provider decision should become clearer, not more complicated.

1

You send the basics

Service, suburb or postcode, timing and one way to contact you. That is enough for us to begin.

2

We check the practical fit

We look at the service you need, where you live and the timing that matters to you, then check what we can realistically provide.

3

We talk with you

If there is a potential match, we contact you to understand the situation properly and confirm what matters before anything is scheduled.

4

We plan before we promise

Only after the service, people, timing and relevant approval details line up do we move into service planning and onboarding.

Ready to choose

Let’s get started. We’re here to help.

Send us the basics and we will take it from there. You do not need to have every answer or know the right aged-care wording before you contact us.

What happens after you send this

One of our team will read your enquiry, look at the service, area and timing, and get back to you so we can talk it through properly. If we can help, we will work through the next step with you.

Prefer to talk? That is absolutely fine. We would be happy to hear from you.

If you would like, tell us a little more. All we need to get started is your name, a way to reach you and your suburb or postcode. The details below can help us check the practical fit more quickly, but you do not need to have everything worked out before we talk.

Before you contact us

A few common questions.

The first step is intentionally light. We only need enough to decide whether a real service-planning conversation makes sense.

What do you need from me to check whether Acclyptic may be a good fit?

Start with the service you have been approved for, your suburb or postcode and the days or times that matter most. If there appears to be a workable match, we can talk through the relevant approval or referral details together.

Do I need to enter a referral code into this form?

No. This first enquiry is designed to be simple. You can tell us whether you have your approval or referral information available, but you do not need to enter a referral code into this website form.

What happens if the timing or service is not available?

We will check the service, location and timing first. If we can help, we will explain the next step clearly. If we cannot, we will tell you rather than leave you waiting on an uncertain answer.

Can I call instead of filling in the form?

Absolutely. The form is simply a quick way to send us the basics, but we are more than happy to talk with you on the phone and work through your situation in your own words. You will not get an answering machine or a “press 1 for this, 2 for that” phone maze — when you call Acclyptic, you will speak directly with a real person who can listen and help you work out the next step. No problem at all.

Not actually at this stage?

That is okay. Start from where you are.

If you are still waiting, want to change provider, need a CHSP service match or are only beginning to explore aged care, use the broader next-step guide.
